Vehicle Accident Kit

• Vehicle Accident Kit Folder - Print on 8 1/2" X 11" paper and fold in half to create a folder for the Vehicle Accident Kit inserts.

o       Vehicle Accident Kit Folder

• Vehicle Accident Kit Inserts - Place one of each of the following accident report forms or inserts inside the Vehicle Accident Kit Folder.

o       What to do In Case of an Accident and Contact Information Insert – Includes two inserts to be printed on one 8 1/2" X 11" page.  The 1st insert summarizes the steps to take when an accident occurs and the 2nd references LGIT's address, phone and fax numbers.  Cut in half before inserting these inserts into the Vehicle Accident Folder.

o       Accident Scene Report – A report filled out at the scene of the accident by the driver or local government representative.  Fold in half before inserting into the Accident Kit Folder.

o      Witness Accident Questionnaire – A report filled out by witnesses at the scene of the accident. Print out on 8 1/2" x 11" card stock and cut into individual cards.  Place two cards in each Vehicle Accident Kit Folder.

o       Quick Reference for Proper Reporting of New Claims – A document checklist for the person responsible for reporting the new claim.  Fold in half before inserting into the Vehicle Accident Kit Folder.

o       Vehicle Accident Report (PDF format) (RTF format) – A claims 1^st Notice of Loss report filled out by the person responsible for reporting the new claim. Fold in half before inserting into the Vehicle Accident Kit Folder.

o       LGIT Supervisor’s Accident Investigation Report – A report filled out by the department supervisor upon investigation of the accident. Fold in half before inserting into the Vehicle Accident Kit Folder.
