What to Do in Case of an Accident and Contact Information Insert - This includes two inserts to be printed on regular 8.5-inch by 11-inch piece of paper. The first insert summarizes the steps to take when an accident occurs and the second references LGIT's address, phone and fax numbers. Cut in half before inserting these inserts into the Vehicle Accident Kit folder.
Accident Scene Report - This is a report to be filled out at the scene of the accident by the driver or local government representative. Fold in half before inserting into the Vehicle Accident Kit folder.
Vehicle Accident Report (PDF) (RTF) - This claims first notice of loss report is to be filled out by the person responsible for reporting the new claim. Fold in half before inserting into the Vehicle Accident Kit folder.
